Written by John Sapcote, whose name is thus inscribed on the first page: "Liber Johannis Sapcote, Londini emptus decimo octavo die mensis Octobris, anno humanæ salutis reparatæ 1547 currente."

The forms extend in date from the time of Hen. VIII. to that of Q. Mary, some of the later being in the name of Card. Pole.

(fols. 23–32) appear to be written by another hand, and an index has been added by a hand of the following century.
