Written in 1641–1649, by Edward Bastard, begun 27 July, 1641, with the following heading on fol. 9: "A booke of notes and observacions taken by me T. B. in diverse cases: capt. in anno 1641 et postea."

Prefixed on fol. 1 are receipts for horse-medicines, including one for a horse "sicke of the new dissease"; on fol. 2, a drink for the stone and "A speciall medicine for the plague; probatum; I had this of Mrs. Pratt, 24 July, 1647"; on fol. 8b, two receipts for black ink.

On the cover is an acknowledgment by Cuth. Brereton of borrowing "one book of presedents in fo." from Edmond Hear on 1 Oct. 1667, to be returned before Christmas.

The book is stained by damp.
