Recovered from a folio volume of miscellaneous printed church music bound together for Sir Frederick Ouseley in 1855. It consists of two elements, originally separate: (a) fols. 1-15 in the hand of an unknown scribe and (b) fols. 16-79 transcribed in 1847 by George Townsend Smith, organist of Hereford Cathedral, who has added notes on the sources of his transcriptions.
